This piece is a promotional item which includes a set of forms and diagrams to help a physician determine the severity of a psoriasis patient’s condition.

Consisting of 16 pages, including covers and tabs, the sheets are constructed from heavy card, held together with three metal rings. Four tear-pads are held in slits on their appropriate pages.
